CHAPERONE RESPONSIBILITY

Sign and return this form to the office before you chaperone a field trip.  Once filed in the office, it will be good for the entire year. 

I am at least 18 years old.

I understand that I am responsible for the safety of Drought School students in my care as I chaperone their field trip.

I understand that I represent Drought School and will conduct myself appropriately.

I further understand that in the event of an accident, my insurance is the primary, and the district’s liability umbrella follows 2^nd.

I will not use alcohol or drugs (including cigarettes) from the time we leave the Drought School grounds until the time we return to the Drought School grounds.
